• Complete your employment application and return it to the Department for Human Resources.

• Completed applications will remain active for one year.  It is your responsibility to notify us in writing, at one-year intervals, of your interest to remain an active candidate for employment.

• Falsification of information or failure to fully disclose information may result in rejection of the application or dismissal if you become employed.  All applicants should refer to the Felony Record Search (See page   ) regarding arrests, convictions and other court adjudications as listed therein.

• Please print in black ink or type.

• Your application is complete when all of the following items are received in Human Resources:
  

  • Completed Application Form

  • Verification of High School Diploma, or

  • Complete Official Transcript(s) of all college credits

  • Copy of your Social Security card with correct name

  • Minimum of three (3) work-related references including most recent employment supervisor or principal
