active for one year. It is your responsibility to notify us in
writing, at one-year intervals, of your interest to remain an active
candidate for employment.

failure to fully disclose information may result in rejection of the
application or dismissal if you become employed. All applicants should
refer to the Felony Record Search (See page ) regarding arrests,
convictions and other court adjudications as listed therein.

of the following items are received in Human Resources:

Verification of High School
Diploma, or

Complete Official Transcript(s)
of all college credits

Copy of your Social Security card
with correct name

Minimum of three (3) work-related
references including most recent employment supervisor or principal

Certification and Training for
Instructional Personnel must be eligible for a Oklahoma Teacher's
Certificate Application for the certificate should be submitted to the
Oklahoma State Department of Education in Oklahoma City, Oklahoma.
Copies of any communication received from the Department of Education should
be forwarded to Human Resources.
