1. Download the selection instrument and save it to your computer. When you use the Save As command, rename the document to your school name.

 2. Once you have saved the document on your computer you will be able to fill it out and email it back to Dr. Jeter. To open the document you will need to have Adobe Acrobat Reader 8.0 installed on your computer. If it is not already installed,  call your STC or ISS and they will do the install.
