The application in the links below is the form needed to request the use of
Tulsa Public Schools' facilities. You may submit the online
application or print a copy of the form. If you use the printed
form, please complete and
sign. Before signing/submitting either application, please read the
terms and conditions stated in the form. (For 'the rules and regulations of the Board of Education governing the use of
school buildings', see the School Board Policy on Community Use of
School Facilities by clicking the "Board Policy" link to the left.)
Once completed, the printed application should be submitted to
the school requested or you may submit it to the Facilities Utilization
Department. The online application will be routed directly to the
Facilities Utilization Department. Any application submitted to the
Facilities Utilization Department must be sent to the requested school for
the initial approval before being processed. Please note:
Applications must be approved by school administration and in the Facilities
Utilization Office no later than 10 days before the event.
