Beginning at age 14, the IEP must identify and address the special education student's post school preferences and interests.  This should include information about where the student wants to live, learn, work, and how they wish to become involved in the community.  The team should consider the type of college, technical/business school, or other academic environment the student wishes to attend after completing the secondary program.  Information on the IEP must document the student's referral to a vocational rehabilitation counselor no later than age 16.
 

Examples of Transition Statements

Susan will participate in a curriculum that addresses functional daily living skills to allow her to live independently after completing high school.
James will take entry level courses in computer education to prepare him for participation in coursework at a technical college. 
Tonya has not identified any post high school goals.  She will complete career inventories and coursework to help her identify goals and options for her post high school life.
