ENROLLMENT REQUIREMENTS:

            Birth Certificate: Child must be 5 years old on or before September 1, 2008.

            Immunization Records:            4 DPT(Diphtheria-Pertussis-Tetanus) **

                                                          3 Polio (Poliomyelitis) **

                                                          1 MMR (Measles, Mumps, Rubella)

                                                          2  Hepatitis A

                                                          3  Hepatitis B

            Social Security Number

            Proof of Address

A current phone number must be on file where a parent can be reached. If a child becomes ill at school, we must have someone to contact in order for he/she to be picked up. If your phone number changes during the year, please be sure to notify the school.

Please avoid picking up your child before class is dismissed. Also, it is essential that your child be here on time so he/she does not miss the important morning activities.

If your child is going to be absent, call the school office between 8:00 and 8:30 a.m. When you call, give your child’s name, teacher’s name and reason for absence.

Parents often wish to visit their child’s class. We extend a cordial invitation for you to do so. Please call before you come. In order for us to know who is in the building, we require you to come by the office prior to going to the classroom. Because visitors affect all children, we ask that other children not be brought to visit and that visits be limited to 30 minutes.

Parents are encouraged to become actively involved in the school operations. Numerous opportunities are made available through the Chouteau PTA, Chouteau School Planning Council and the Volunteer Program. From time to time the 4 year-old program teacher will need volunteers for the classroom. Please consider saying; "YES" when asked to help.
