IMMUNIZATIONS

          The clinic is responsible for maintaining your child’s

immunization records and to make sure that all students are in compliance with Oklahoma State laws.

Requirements are: 

DPT --- 5 doses (4 if the 4^th dose was given

              after the 4^th birthday)

OVP --- 4 doses (3 if the 3^rd dose was given 

               after the 4^th birthday)

MMR --- 2 doses (1^st dose must have been

               given on or after the 1^st

               birthday)

HEP A -- 2 doses

HEP B -- 3 doses

VARICELLA -- 1 dose or have had the disease
