Parent & Student Handbook
Age Requirements
Students entering Kindergarten must be five years old on or before
September 1.
Enrollment Requirements
1. Proof of address
a. Current utility bill
b. Professional mail, which includes a bank statement,
doctor's bill, a letter from DHS, letter from an employer,
mail from a law firm
c. Typed rent lease with landlord's phone number
DRIVER'S LICENSE WILL NOT BE ACCEPTED
2. If the student is new to TPS,
a. the immunization record is required
b. birth evidence must be presented (this includes the
original birth certificate)
3. Social
Security Card
4. Please provide the address of the last school your child
attended if it's a school other than a Tulsa
Public School
Immunization
Requirements
Oklahoma law requires that all public school children be immunized
against diphtheria, pertussis, tetanus, measles,
mumps, and poliomyelitis. Immunization records are required for
enrollment.
School Hours
School hours are 8:05 a.m. until 2:50 p.m.
Office
Hours
Office hours are 8:00 a.m. until 4:00 p.m.
Leaving
School During the Day
Students may not be sent from the school grounds by any person for any
reason without the permission of the office. The office must secure the
parent's consent or notify them before this is allowed.
Students are to
be checked out in the office by their parents.
Students who
become too ill to remain in class will be given a pass to the school
nurse. No student will be excused from school or leave the school because
of their illness without the permission of the school nurse or
administration.
The building
principal must approve all field trips. No field trips will be approved
where students are transported via private
automobiles. Students must have signed permission slips to
attend.
Absences
If your child is going to be absent from school, please call the office
(918) 833-8960 between 8:00 a.m. and 9:00 a.m.
If we do not receive a call, we will attempt to call the parents
to verify the absence.
Tardiness
Students are expected to be in the Assembly Room by 8:05a.m. When a
student is tardy three times consecutively or continues to be late
on a regular basis, they will be referred to the school counselor for
appropriate action.
Emergency Care for Ill or Injured Children
If a child becomes ill or injured at school, every reasonable effort is
made to care for them. If the parent, the listed responsible friend,
relative, neighbor, or the physician cannot be reached, the child will be
taken to the emergency room or an emergency care center of the hospital
listed on the child's school records, or from the recommendation of the
nurse in the doctor's office. TULSA PUBLIC
SCHOOLS DO NOT ASSUME RESPONSIBILITY FOR THE PAYMENT OF HOSPITAL OR
DOCTOR'S FEES.
Medication
Tulsa Public
School permits administration of medication during the school day only
with written directions from the physician
and the parent. If your physician feels it is necessary to
administer medication during school hours, you will need to fill out a
"Authorization for the Administration of Medication by Designated
School Personnel". All medications must be brought to the nurse's
office in prescribed containers. All medication should be delivered to
the school nurse by a parent or guardian.
Children are not to transport medication. If you have any
questions, call the Roosevelt Health Clerk at 833-8970.
Cafeteria
Breakfast and lunch is provided to all students free of charge.
Breakfast will start at 7:35 a.m.
Lost and
Found
Lost and found clothing items are located in the cafeteria. Other items
may be recovered in the office.
Code of
Conduct
Good discipline is the key to a healthy, happy learning environment.
Proper behavior, cooperation, respect for others and self-discipline are
critical elements of a student's educational development. Contact the
Office for a copy of the "Rights and
Responsibilities" pamphlet. Continuous disruptive behavior and
willful refusal to accomplish school tasks, insubordination, disorderly,
vicious, illegal or immoral conduct and persistent violation of school
regulations are causes for suspension.

procedures and disciplinary actions are further outlined in the
Tulsa Public
School Code of Student Conduct
(copies are available in the school office).
Special
Progress Reports and Report Cards
Student Progress reports will be sent to parents the fifth (5th) week of
each nine (9) week quarter if a student is making
unsatisfactory progress. Parents are requested to sign the
report and return it to school the following day. This is our way of
knowing that you received the report.
Report cards are
issued eight (8) days following the end of the first quarter. Parents are
requested to sign these and return to the record teacher the following
day.
Dress
Code and Grooming Guidelines
Roosevelt students will be in uniform dress code for the 2007-08 school year. They
will wear Royal Blue, Orange or White Polo shirts and Navy Blue and
Khaki pants/skirts.
