9th - 12th Grade
DTP/DTAP/TD - 3
Polio - 3 MMR -2
Hep. A - 2
Hep. B - 3
Varicella - Immunization or history of disease

According to state law, students MUST meet these minimum requirements to remain in school. Students may be eligible to receive needed immunizations at school. Contact school nurse for eligibility.
